Prevention of Human Factor Failure in Hazardous Activities

The diploma thesis deals with the topic of human failures during the risk activities. The aim of the work is to apply risk analysis to the operation of a selected unnamed company that processes vegetables. The thesis is divided into three parts. In the first part, the theoretical basis is discussed and serves as a basis for a better understanding of the topic and subsequent elaboration of the analytical part. The analytical part focuses on the characteristics of the selected company, organizational structure, working positions and the operation itself and analyses these issues with the focus on human factor. The last part of the diploma thesis is dealing with suggestions leading to the prevention of human failure in risk activities. The aim of these suggestions is to improve the situation of the company.
Risk Management inside Construction Company

The aim of the thesis is to analyse issues of risk management in a construction company, especially in the context of construction contracts implementation. The first part is devoted to applied terminology and interpretation of relevant terms, followed by a classification of risks from different perspectives, including the risks’ possible further sub classification. Furthermore, significant risk resources and construction participants who might be affected by the risks are presented. The third chapter deals with risk management; individual phases of this complex process are described herein, including their correct sequences, as well as a selection of methods and techniques that are standardly used in risk management. The last, fourth chapter summarises the problems of risk management on a practical example, where a construction company implements a construction contract by applying selected risk management methodologies.

Construction Order Risk Management

The subject of the bachelor thesis is the issue of risk management in the construction company. The first part of the thesis is based on the terminology used and the interpretation of the relevant terms. Here we can find the classification of risks from different angles of view, possible division and classification of risks. Then there is a focus on significant sources of risk and on those involved in construction that are affected by the risks. The fourth chapter is to familiarize with the different stages of the risk management process, including the introduction of selected methods and techniques that are commonly used. The theoretical part is the basis for the fifth chapter, which summarizes the problems of the risks of a particular construction company and two of their construction orders.
Pressure ulcer as an adverse event

The bachelor thesis "Decubitus as an adverse event" deals with the comparison of the modified Norton scale and the Braden scale. Furthermore, the thesis deals with the frequency of decubitus in the Central Military Hospital in Prague. The aim of the work was to compare the accuracy of both scales. Another aim of the work was to monitor the preventive measures introduced. Part of the work was to determine and describe the frequency of occurrence of decubitus in the selected medical facility. The theoretical part of the thesis explained the basic definitions and essential concepts related to decubitus, adverse events and departmental safety objectives. The empirical part contains the results of the actual research investigation and its comparison with the results of studies focusing on similar topics. Method: a mixed methods research design was chosen. Results: 48.48% of the respondents rated the risk of decubitus according to both scales in agreement. In 51.52 % respondents the evaluation according to one scale did not correspond to the evaluation according to the other scale. The Braden scale had a sensitivity of 100% and specificity of 59%. Compliance with departmental safety objectives in surgical wards

Abstract Introduction: The Ministry of Health of the Czech Republic defined the departmental safety objectives in 2010 and their purpose is to minimize the risks of the most common adverse events that may occur during the provision of health care. Aims: The main aim of the presented work is to evaluate the observance of departmental safety goals by nurses in surgical wards in selected hospitals in the South Bohemian region. Methodology: To achieve the goals, a qualitative research method was chosen, which took place in the form of in-depth interviews. Communication partners were selected by deliberate selection. 14 individuals took part in the anonymous interviews. The research group consisted of senior, station, general and practical nurses working in surgical wards in selected hospitals in the South Bohemian Region. Results: The results of the research survey showed that the interviewed nurses working in surgical wards do not proceed uniformly in the provision of care in areas that define departmental safety goals. This concerned mainly the identification of patients, safety preoperative procedure, and the introduction of preventive measures to minimize the risk of falls and pressure ulcers. The results of the research also confirmed that nurses can provide better quality care with regard to patient safety, especially in the way of patient identification, administration of higher-risk drugs, prevention of nosocomial infections and ordering of medication by telephone. Furthermore, the results of the research survey show that nurses know and appropriately use preventive measures in the field of prevention of falls and pressure ulcers. Conclusion and recommendations for practice: Health care is inherently linked to the risk of error, so it is important that nurses provide safe care. The output of the diploma thesis is a proposal for systemic changes in the field of compliance with departmental safety objectives intended for nursing management.
Managing the change in the reporting system of adverse events at Žatec Hospital

IN ENGLISH The aim of my thesis is to introduce the system of monitoring of undesirable events and its relationship with the legislative regulations of the Czech Republic and outline the process of controlled change in the organization. The theoretical part of the thesis is dedicated to the general description of the system of reporting undesirable events at national and international level and is based on research from available literature. The other important part of the theoretical part of the thesis is dedicated to the quality system and basic processes of controlled change processes as described by the Czech and foreign authors. The practical part of the thesis is based on a case study from The Žatec Hospital, which represents the initial analysis of the monitoring system of the undesirable events in the organization, planning and description of the processes and individual steps in the process of controlled change with subsequent analysis of the system of adverse event monitoring after aforementioned change. Furthermore the results of the original and the current system comparison are being presented.
